Job summary

HarmonyCares Primary Care Medical Group in Detroit seeks a Value Based Care Physician Leader to guide a growing team. The role partners with Physician Leaders and Regional Leaders to drive HEDIS outcomes and quality-focused care for high-risk Medicare patients Monday through Friday.

As Medical Pod Leader you will oversee clinical pod performance, foster teamwork, and collaborate with APPs, Market Manager and office staff to enhance care at home for complex patients.

Qualifications

• Must be a Doctor of Medicine or Osteopathy.
• Active CPR Certification.
• Unrestricted DEA.
• Completion of an accredited Residency Program.
• Board certification is required for all newly hired or promoted on or after January 1, 2026; must obtain and maintain certification in accordance with organizational and applicable regulatory standards.
• 3 years of experience as a Physician collaborating with Advance Practice Providers.
• Experience in Geriatrics or complex patients.

Responsibilities

• Lead the clinical pod's performance including accurate diagnosis, documentation, quality, patient experience, affordability, patient safety, and provider productivity.
• Develop and maintain close working relationships with all clinical pod members.
• Serve as the collaborating physician for associated pod advanced practice providers (APPs).
• Maintain a dyad partnership with the Market Manager and market office staff.

Job Description

The Medical Pod Leader will be the clinical leader responsible for the clinical performance of clinical pod team members – to include: Accurate diagnosis and documentation, quality, patient experience, affordability, patient safety, clinical team dynamics, and provider productivity. The pod leader will be tasked with developing and maintaining close working relationships with all clinical team members in the clinical pod. The pod leader will also function as the collaborating physician for associated pod advanced practice providers (APPs). This position will have a dyad partnership with the Market Manager and market office staff.

Required Knowledge, Skills and Experience

• Must be a Doctor of Medicine or Osteopathy
• Must maintain a valid driver’s license and maintain a good driving record
• Active CPR Certification
