Job summary

HarmonyCares in Troy, Michigan seeks a Value-based Physician Leader to guide a growing Medical Pod and supervise APPs. You will balance chronic disease management with leadership of high-risk clinical huddles and oversee clinical oversight activities.

The role entails collaborating with market leadership, driving quality metrics, and maintaining strong relationships with the clinical team to deliver patient-centered home-based care.

Qualifications

• Bachelor’s Degree required.
• Doctor of Medicine or Osteopathy required.
• Completion of an accredited Residency Program.
• Unrestricted Medical License required.
• Board certification required for hires after Jan 1, 2026.

Responsibilities

• Act as on-site supervising Physician for assigned APPs and collaborating physicians.
• Collaborate with clinical team members and support APP development.
• Lead high-risk huddles and ensure care plans include all team members.
• Review quality metrics and pod performance to drive improvement.
• Discipline and performance reviews for direct reports.
