Qualifications

• MD or DO degree required and board-certified in Internal Medicine, Family Medicine, Geriatrics, or Hospital Medicine.
• Current unrestricted medical license in the state of employment.
• Active DEA license required.
• Proficiency with EMR systems and clinical documentation.

Responsibilities

• Welcome new residents with thorough assessments and establish continuity of care for your patient panel.
• Deliver comprehensive care across chronic disease management, preventive services, and acute needs with clinical judgment.
• Round regularly to maintain connection with patients and proactive care.
• Collaborate with facility staff, families, and care teams to create personalized care plans.
• Guide residents and families through health decisions and resources to support independence and quality of life.
• Maintain accurate EMR documentation and comply with regulatory standards and Curana policies.
