Job summary

AlignMed Partners seeks an Advanced Practice Provider to deliver primary and urgent care to nursing home residents in Burlington, NJ. The role supports collaboration with center leadership and medical directors to ensure high-quality patient care.

The candidate should have an NP or PA background with senior care experience, and be ready to work in a fast-paced post-acute setting, with access to UpToDate and other clinical resources.

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Licensed as a registered nurse (RN).
• Certified as a Nurse Practitioner with a Master’s in Nursing and a focus in Older Adult and/or Family NP.
• PA must be graduate of ARC-PA accredited program and hold PA license with NCCPA certification.

Responsibilities

• Provide primary and urgent care services to nursing center residents.
• Collaborate on-site with Center Leadership and Medical Director for patient care.
