Job summary

Personalized Health Partners is seeking a Nurse Practitioner to deliver high-quality primary care at Glenwood Healthcare Center, focusing on seniors in a high-touch care model.

The NP will diagnose, treat, and manage acute and chronic conditions, collaborate with PCPs and care teams, and engage residents and families in care planning. Schedule is Monday–Friday, daytime, with no weekends or on-call, and facilities-based care to reduce hospital visits.

Qualifications

• Must have MS in Nursing, DNP or NP certificate.
• National certification by AANP or ANCC.
• Active DEA license.
• 2+ years clinical experience in geriatric, adult or family practice.
• Passionate about healthcare for seniors and low-income populations.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Develop treatment plans and communicate them to residents and providers.
• Order and interpret lab and diagnostic tests; adjust treatment plans.
• Maintain compliant documentation in EMR software.
• Collaborate with operations and leadership to achieve goals.
• Support value-based care initiatives.
• Provide excellent customer experience to residents and families.
• Follow regulations and standards of practice.
• Coordinate care with PCPs, specialists, and telemedicine providers.
• Complete monthly/quarterly/annual visits as needed.
• Participate in family conferences and advance care planning.
