Job summary

Golden Valley Health Centers is seeking a PACE Activities Specialist for the Modesto center. The role focuses on developing and facilitating recreational groups to meet social, emotional, cognitive, and physical needs of participants.

The position requires coordinating with the care team, assessing participants, documenting progress, and creating diverse activity programs. Schedule is Monday–Friday, 8:00am–5:00pm, with competitive benefits.

Qualifications

• Valid California driver’s license and vehicle insurance.
• Ability to work effectively in a team environment.
• Strong understanding of psychosocial problems of aging.
• Ability to act within the scope of authority to practice.
• Ability to interact respectfully with geriatric individuals including those with cognitive decline.

Responsibilities

• Conducts initial and reassessments for participants, at least every six months.
• Develops therapeutic programs with participant input as part of care plan.
• Records and dates progress notes in the medical record.
• Creates diverse activity formats: small groups, one-on-one, and large groups.
• Designs activities to restore self-care and independence.
• Posts a schedule of activities in a central, accessible location.
• Participates in Interdisciplinary Team meetings and care planning.
• Observes participant changes and reports to the nurse.
• Coordinates volunteer activities and serves as a staff resource.
• Fosters a welcoming community and supportive social atmosphere.
• Engages in QA activities and coordinates with community resources.
• Organizes activities to restore functioning and independence.
• Weekly consults with occupational therapist or recreation therapist.
