Job summary

Family Tree Private Care is seeking a Care Manager to support older adults, people with disabilities, and families by assessing needs, creating care plans, and coordinating services across home, hospital, and community settings. You will serve as the primary contact, navigate complex care options, and advocate for client safety and independence while maintaining confidentiality and professional boundaries.

Role requires travel to clients' homes across Houston and surrounding areas, driving

Qualifications

• Bachelor’s degree in social sciences, social work, or related field.
• 1+ year in a clinical setting or social work.
• 1+ year experience with patients or client relationships.
• Sales-driven experience is a plus.
• Geriatric management expertise is a plus (Aging Life Care).

Responsibilities

• Complete and document initial assessments of client needs and goals.
• Develop care plans with clients and representatives reflecting preferences.
• Reassess regularly and update care plans as needed.
• Coordinate home, nursing, medical appointments, and therapies.
• Attend appointments, explain recommendations, and follow up.
• Support transitions from hospitals or facilities and coordinate changes in care level.
• Advocate for client safety and preferences; respond to urgent concerns.
• Educate families on aging, cognitive changes, and care options.
• Provide timely updates to authorized contacts with consent.
• Maintain confidential client records and billable activities.
• Participate in team meetings and quality improvement activities.
• Develop community outreach and referral relationships.
