Job summary

Habitat Health is seeking a Home Care Coordinator to ensure in-home and clinical homecare needs are delivered to help participants thrive. You will work with an Interdisciplinary Team (IDT) to assess needs, develop care plans, and coordinate home care services in the participant's home setting.

The role requires clinical experience, strong judgment, and excellent communication skills. Travel between community locations is expected; a valid driver’s license and reliable transportation are

Qualifications

• Minimum of two years of clinical experience in home care, geriatrics, or care coordination.
• Ability to conduct comprehensive assessments and identify care needs.
• Strong communication and collaboration within an interdisciplinary team.

Responsibilities

• Conduct in-home assessments to evaluate care needs, functional status, safety risks, and appropriateness of home care services.
• Develop and update participant care plans with the IDT.
• Coordinate home care services to align with the individualized care plan and goals.
• Maintain current authorizations and ensure payer requirements are met.
• Review weekly home care notes to identify service gaps and follow up as needed.
