Job summary

Family Tree Private Care is seeking compassionate Registered Nurses for in-home care across Texas. We offer full-time, part-time, and PRN opportunities with flexible scheduling in patients’ homes.

RNs will travel to patient residences, assess conditions, manage care plans, and educate families. A valid RN license and driver’s license are required; lifting up to 25 lbs and at least 1 year in home health is preferred.

Qualifications

• Minimum 1 year of home care experience (or related healthcare field).

Responsibilities

• Travel to assigned patients' homes and manage care plans per physician instructions.
• Assess and chart patient condition at each visit.
• Review medications and vital signs during evaluations.
• Dress wounds and monitor healing progress.
• Educate caregivers and family on aftercare and ongoing care.
• Provide comfort through palliative care.
• Recommend devices to improve quality of life.

RESPONSIBILITES

• Travel to assigned patient’s homes and manage their care plan according to physician’s instructions.
• Assess and chart observations of the patient’s condition at each visit.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Dress or redress wounds and assess the healing progress.
• Educating caregivers and family on the aftercare or ongoing care of the patient.
• Keep the patient comfortable through palliative care.
• Recommend tools or devices that may improve the quality of life for the patient.

SKILLS

• Strong interpersonal skills that allow them to get to know their patients, build personal rapport and establish trust to provide high-quality care while increasing patients’ enjoyment and comfort.
• Initiative so they can make independent decisions when required and accept direction and requests from patients.
• The ability to follow instructions but also act independently if necessary.
• Excellent observational and problem-solving skills
• Ability to act swiftly in emergencies and work efficiently.
• Flexibility and patience to work in a variety of living environments.
• Knowledge of clinical skills including changing bandages, giving tube feedings, administering shots, and setting up and monitoring IVs

REQUIREMENTS AND QUALIFICATIONS

• Relevant license and certification
• A valid driver’s license and reliable transport
• Ability to lift 25lbs
• Strong candidates have a minimum of 1 year of experience working in the home care industry (or related industry – hospice, home health, hospital, etc.)
