Job summary

Unicity Care Management in New Jersey is seeking a Care Manager to oversee a small private-pay caseload, coordinating services, monitoring client progress, and providing in-home support across Independent Living, Assisted Living, or Memory Care settings.

The role requires licensure as a Social Worker with 7–10 years in geriatrics, a valid driver's license, and the ability to work with minimal supervision. Hybrid schedule and strong collaboration with referral sources are offered.

Qualifications

• Licensed Social Worker with 7-10 years of experience working in geriatrics.
• Ability to work with minimal supervision.
• Valid Driver’s License and dependable personal vehicle.

Responsibilities

• Perform in-depth client assessments across medical, psychosocial, environmental, financial, legal, and family domains.
• Conduct Fall Risk, MMSE, and Depression screenings for each client.
• Coordinate all service needs and develop care plans with client and family.
• Collaborate with hospital, nursing home, AL/memory care staff, physicians, and other professionals.
