Job summary

Qualifications

What You'll Do:
  • Conduct comprehensive client assessments and develop personalized care plans that support health, independence, and quality of life
  • Coordinate care and connect clients with healthcare services, community resources, and supportive programs tailored to their needs
  • Serve as a trusted advocate for clients and families, helping them navigate healthcare decisions and access appropriate services
  • Collaborate with physicians, healthcare providers, caregivers, and community partners to ensure seamless communication and care coordination
  • Support clients through care transitions, including hospital discharges and placement planning for rehabilitation, assisted living, or skilled nursing settings
  • Provide ongoing guidance, education, and support to clients and families while monitoring progress and adjusting care plans as needs evolve
What You'll Need:
Required:
  • Registered Nurse of Licensed Clinical Social Worker (LCSW/MSW) or equivalent human services professional
  • 3+ years of experience in geriatric care management, aging services, home care, case management, or related field
  • Strong knowledge of community resources, healthcare systems, and long-term care options
  • Valid driver's license and reliable transportation
Preferred:
  • Care Management Certification (CMC) or Aging Life CareProfessional® certification
  • Knowledge of Medicare, Medicaid, and long-term care planning
  • Experience working with older adults and individuals with chronic illnesses, dementia, or disabilities
