Unicity Care Management, based in New Jersey, seeks a Social Work Care Manager and Patient Advocate to guide families through critical healthcare decisions. You will manage a small, private-pay caseload with autonomy and direct contact with clients, including crisis support.

The role blends field visits with a hybrid schedule, offering a base salary of $75,000–$85,000, mileage reimbursement, and a full benefits package including health/dental/vision coverage and a 401(k) plan.

Qualifications

• Active Licensed Social Worker (LSW or LCSW) in New Jersey.
• Genuine interest in working alongside families.
• Strong written and verbal communication.
• Organizational discipline to manage an independent caseload and maintain documentation.
• Collaborative approach to working with physicians, providers, and community partners.

Responsibilities

• Conduct comprehensive in-home assessments covering functional, cognitive, psychosocial, and environmental needs.
• Develop and maintain individualized care plans, revising them as circumstances change.
• Coordinate physicians, home care, specialists, and community resources into a single coherent plan.
• Support clients and families through transitions of care, closing gaps where people are often lost.
• Provide emotional support and crisis intervention at moments families need it most.
• Advocate for clients at appointments, care conferences, and points of decision.
• Serve as the primary point of contact for the clients on your caseload, including urgent needs outside standard hours.
• This is a field-based role requiring regular travel and is not an in-office position; provides high-touch, concierge-level assistance to clients.
