What You'll Do:
- Conduct comprehensive client assessments and develop personalized care plans that support health, independence, and quality of life
- Coordinate care and connect clients with healthcare services, community resources, and supportive programs tailored to their needs
- Serve as a trusted advocate for clients and families, helping them navigate healthcare decisions and access appropriate services
- Collaborate with physicians, healthcare providers, caregivers, and community partners to ensure seamless communication and care coordination
- Support clients through care transitions, including hospital discharges and placement planning for rehabilitation, assisted living, or skilled nursing settings
- Provide ongoing guidance, education, and support to clients and families while monitoring progress and adjusting care plans as needs evolve
What You'll Need:
Required:
- Registered Nurse of Licensed Clinical Social Worker (LCSW/MSW) or equivalent human services professional
- 3+ years of experience in geriatric care management, aging services, home care, case management, or related field
- Strong knowledge of community resources, healthcare systems, and long-term care options
- Valid driver's license and reliable transportation
Preferred:
- Care Management Certification (CMC) or Aging Life Care Professional® certification
- Knowledge of Medicare, Medicaid, and long-term care planning
- Experience working with older adults and individuals with chronic illnesses, dementia, or disabilities
