Job summary

Unicity Care Management seeks a Social Work Care Manager and Patient Advocate to provide high-touch, field-based care coordination for families facing medical decisions. You will conduct in-home assessments, build and adjust care plans, and coordinate multiple providers to create a seamless, client-centered approach.

You will advocate for clients, offer emotional support, and act as the primary contact for your caseload, while traveling to visit clients and delivering concierge-level nursing

Qualifications

• Active Licensed Social Worker (LSW or LCSW) in New Jersey.
• Genuine interest in working with families, not only clients.
• Sound professional judgment in community settings.
• Strong written and verbal communication; clear voice for families under stress.
• Organizational discipline to manage an independent caseload and documentation.
• Collaborative approach with physicians, providers, and community partners.
• Valid driver’s license and reliable transportation for field visits.

Responsibilities

• Conduct comprehensive in-home assessments covering functional, cognitive, psychosocial, and environmental needs.
• Develop and maintain individualized care plans; revise as needed.
• Coordinate physicians, home care, specialists, and community resources into a single plan.
• Support clients and families through transitions of care; close gaps.
• Provide emotional support and crisis intervention when needed.
• Advocate for clients at appointments, care conferences, and decisions.
• Serve as primary contact for caseload; respond to urgent needs outside hours.
• Field-based role requiring regular travel; high-touch nursing support as needed.
