Job summary

Centers Plan for Healthy Living/Centers Care Solutions in New York is seeking a Care Coordinator to support a multidisciplinary team, schedule member visits, and coordinate treatments under the Managed Long Term Care program. Qualifications

• HS Diploma is required.
• Bachelor’s Degree preferred.
• 2 years customer services experience.
• Strong written and verbal communication skills.
• Computer proficiency.
• Strong multi-tasking abilities.
• Experience in a managed care environment preferred.
• Bilingual preferred.

Responsibilities

• Assists in scheduling of ongoing member visits (UAS Assessments) and tracks rescheduled appointments.
• Must reach daily scheduling quotas.
• Provide a daily report of all scheduled visits.
• Assists with uploading all UAS documents into member files accordingly.
• Generates member correspondence and handles mailings as needed.
• Coordinates communication between Field Assessors and the care team.
• Documents daily calls and follows care coordination guidelines.
• Acts as liaison between member/family, physician(s) and the care team for timely scheduling.
• Participates in ongoing education activities.
• Ensures compliance with policies, NY State and Federal regulations.
• Other assigned duties.
