Job summary

UniversaCare Inc. is seeking a Care Coordinator Team Lead in New York to supervise scheduling staff, support care coordinators, and partner with operations leadership.

You will oversee caregiver scheduling, client coordination, and staffing coverage while ensuring high-quality service delivery. Ideal candidates bring 2+ years in home care coordination, strong Excel skills, and experience with HHA Exchange or AxisCare.

Qualifications

• Minimum of 2 years experience in home care coordination, scheduling, or healthcare operations.
• Leadership or supervisory experience preferred.
• Experience with HHA Exchange and/or AxisCare required or strongly preferred.
• Strong Excel skills for reporting and data organization.
• Excellent communication and multitasking abilities.
• Bilingual in English and Spanish is a plus.

Responsibilities

• Oversee caregiver scheduling, shift coverage, and case coordination.
• Monitor open shifts, urgent staffing needs, and staff productivity.
• Coordinate with Recruitment and HR on staffing requirements.
• Ensure timely communication with caregivers, clients and families.
• Participate in weekend on-call rotation to address urgent staffing, caregiver, and client concerns as needed.
• Generate reports and analyze staffing or scheduling trends using Excel.
