Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Professional Counselor (LPC) to join an integrated clinical team in local LTC facilities. You will provide therapy, assessments, and care planning with strong administrative support, Monday–Friday schedules, and autonomy in managing your caseload.

Clinical focus includes geriatric care, collaborative treatment with nursing and social services, and thorough documentation using the GNT tool.

Qualifications

• Active LCPC license
• Valid driver’s license and reliable transportation
• Strong written, verbal, and interpersonal communication skills
• Ability to work independently while collaborating with an interdisciplinary team
• Commitment to timely documentation and high-quality care

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances in LTC.
• Deliver behavioral management and brief, goal-oriented interventions aligning with geriatric care best practices.
• Collaborate with nursing, social services, rehabilitation and medical providers to coordinate treatment.
• Participate in care plan discussions, facility meetings, and case collaborations.
• Maintain complete and timely documentation using GNT.
• Follow ethical, professional, and regulatory guidelines for documentation and care delivery.
• Build professional relationships with facility staff and leadership; act as behavioral health resource.
