Job summary

Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor (LPC) to join our integrated clinical team in long-term care facilities. You will deliver therapy, conduct assessments, and collaborate with nursing and medical staff to support residents’ mental health and quality of life.

This role offers daily autonomy, a Monday–Friday schedule, and strong administrative support, with a focus on compassionate, evidence-based geriatric care for older adults and those with chronic

Qualifications

• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Proven ability to organize workload and maintain clinical documentation.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances.
• Deliver behavioral management and brief, goal-oriented interventions.
• Coordinate treatment with nursing, social services, rehabilitation, and medical providers.
• Participate in care plan discussions, facility meetings, and case collaborations.
• Maintain complete and timely documentation using the GNT.
