Job summary

Deer Oaks Behavioral Health in Texas seeks a Licensed Professional Counselor to join our integrated clinical team on site at local long-term care facilities. You will deliver therapy, assessments, and care planning while enjoying autonomy and strong administrative support.

You’ll collaborate with nursing, social services, rehabilitation and medical staff to coordinate resident care, while maintaining compliant documentation and a focus on geriatric behavioral health.

Qualifications

• Active, unrestricted LCPC license is required.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Provide individual, family, and group therapy to LTC residents and conduct psychosocial evaluations.
• Coordinate care with nursing, social services, rehabilitation and medical providers.
• Maintain complete, timely documentation in the GNT and ensure regulatory compliance.
• Build and maintain partnerships with facility staff and leadership to support resident care.
