• Ability to perform assessments and triage per protocols.

Responsibilities

• Coordinate patient care across a continuum to achieve outcomes.
• Schedule appointments, referrals, and manage provider calendars.
• Assess patient needs telephonically and in person.
• Prescribe/communicate treatment plans and patient teaching.
• Document procedures and ensure equipment is cleaned and stored.
