Job summary

Breakthrough Functional Medicine in Wilmington, NC is seeking a second provider to join our cash-pay, membership practice. You’ll share a panel, conduct foundational assessments, and carry follow-up visits in a root-cause framework that treats the whole person.

You’ll grow with the practice as your scope expands alongside a mentor. We value warm, patient-centered communication, willingness to learn, and the ability to collaborate within a small team.

Qualifications

• Active North Carolina APRN license or ability to obtain before start date.
• Board certification through ANCC or AANP.
• Prescriptive authority with collaborative practice.
• Experience or strong interest in functional/integrative medicine.
• Warm, coachable communication and patient-first mindset.

Responsibilities

• Provide shared-care with a second provider, conducting initial assessments and key milestones.
• Follow-up visits, review progress, and adjust protocols for root-cause care.
• Manage prescriptive care under the collaborative practice agreement.
• Engage members through education, workshops, and community content.
• Coordinate with clinical coordinator and health coach on labs and follow-up.
