Job summary

Aligned Modern Health is looking for experienced Nurse Practitioners to join our Functional Medicine practice in Chicago. Qualifications

• At least two years of experience practicing functional medicine and performing functional diagnostic testing.
• Experience in functional and integrative medicine settings preferred.
• Must have (or be able to obtain) an IL APRN license.

Responsibilities

• Conduct thorough patient assessments including medical history and diagnostic tests.
• Develop personalized treatment plans based on patients' needs.
• Prescribe nutraceuticals and monitor their effectiveness.
• Collaborate with other healthcare professionals.
• Provide counseling on disease prevention and healthy lifestyle choices.
• Maintain accurate and up-to-date patient records.
