OnePeak Medical in Oregon is seeking a passionate Medical Provider to join our growing team. You will provide comprehensive medical services, foster lasting patient relationships, and collaborate with a high-functioning care team to drive optimal health outcomes.

Qualifications

• Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO (or eligibility to obtain).
• Board certification in Family Medicine, Internal Medicine, or applicable specialty.
• Strong interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Comfortable working in a fast-paced, collaborative environment.
• Proficient in using electronic health records.
• Nurse Practitioner Licenses Accepted: FNP, Family Nurse Practitioner
• AGNP- PC, Adult Gerontological Nurse Practitioner (Primary Care)

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment
