Job summary

OnePeak Medical seeks a passionate Medical Provider to join our Southern Oregon clinics in Talent, Medford, Central Point, Grants Pass, and White City. You will provide direct patient care, develop personalized treatment plans, and collaborate with a high-functioning care team to drive optimal health outcomes.

We offer a supportive environment with a 4-day work week, no nights or weekends, and relocation assistance.

Qualifications

• Active Oregon license as NP, PA-C, MD, or DO (or eligibility)
• Board certification in Family/Internal Medicine or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable in a fast-paced, collaborative environment
• Proficient in using electronic health records
• Nurse Practitioner Licenses Accepted: FNP, AGNP-PC

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order/interpret labs and imaging, diagnose conditions
• Develop personalized treatment plans including lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine
• Collaborate with health coaches, nutritionists, mental health providers, and clinical staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment
