OnePeak Medical in Oregon is seeking a Medical Provider to join our growing team. This role thrives in a supportive, forward-thinking environment and focuses on whole-person care within an integrated care model.

You will deliver comprehensive medical services, foster lasting patient relationships, and collaborate with a high-functioning care team to drive optimal health outcomes across our Southern Oregon clinics in Talent, Medford, Central Point, Grants Pass, and White City.

Qualifications

• Active Oregon license to practice medicine.
• Board certification in Family Medicine or Internal Medicine.
• Strong interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Comfortable working in a fast-paced, collaborative environment.
• Proficient in using electronic health records (EHR).

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments.
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions.
• Develop personalized treatment plans including lifestyle interventions and prescriptions.
• Educate and empower patients on preventive health and functional medicine principles.
• Collaborate with health coaches, nutritionists, mental health providers, and clinical staff.
• Document visits in the EHR timely and in compliance.
• Participate in team meetings, quality initiatives, and training sessions.
• Support clinic operations with a positive, team-oriented environment.
