You will provide comprehensive medical services, foster long-term patient relationships, and collaborate with a high-functioning care team to drive optimal health outcomes.

Qualifications

• Active Oregon license as NP/PA-C/MD/DO or eligibility
• Board certification in Family Medicine or Internal Medicine
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records

Responsibilities

• Provide direct patient care through office visits and telehealth
• Perform comprehensive assessments, order labs/imaging, diagnose conditions
• Develop personalized treatment plans including lifestyle interventions and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with health coaches, nutritionists, mental health providers, and staff
• Document visits in the EHR promptly
• Participate in team meetings, quality improvement initiatives, and training
• Support clinic operations and maintain a positive team environment
