OnePeak Medical is redefining primary care with a personalized, integrative model blending traditional and functional medicine. We seek Nurse Practitioners and Physician Assistants to deliver patient-centered care across our Oregon clinics, in a supportive environment.

Qualifications

• Active Oregon license as NP/PA/MD/DO or eligibility.
• Board certification in Family or Internal Medicine.
• Strong interest in preventive, functional and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Proficient with electronic health records.

Responsibilities

• Provide direct patient care through office visits and telehealth.
• Order and interpret labs/imaging; diagnose conditions.
• Develop personalized treatment plans including lifestyle interventions and therapies.
• Educate patients on preventive health and functional medicine principles.
• Collaborate with health coaches, nutritionists, mental health providers and staff.
• Document visits in NextGen EHR in a timely manner.
• Participate in team meetings, quality improvement, and training sessions.
• Support clinic operations with a positive, team-oriented environment.
