Qualifications

• Active Oregon license as a Nurse Practitioner, PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Proficient in using electronic health records
• Comfortable working in a fast-paced, collaborative environment

Responsibilities

• Provide direct patient care through office visits and telehealth
• Perform comprehensive assessments and order/interpret labs and imaging
• Develop personalized treatment plans including lifestyle interventions and medications
• Educate patients on preventive health and functional medicine principles
• Collaborate with health coaches, nutritionists, mental health providers, and staff
• Document visits accurately in the EHR and participate in quality initiatives
• Support clinic operations and contribute to a positive team environment
