Job summary

OnePeak Medical Providers in Oregon is seeking a Nurse Practitioner for Primary Care Functional Medicine. You will provide comprehensive medical services, build lasting patient relationships, and collaborate with a high-functioning care team to drive health outcomes.

This role offers guaranteed salary with earning potential of $300k+ and relocation assistance. Enjoy a 4-day work week with no nights or weekends, plus strong clinical support and ongoing functional medicine training.

Qualifications

• Active Oregon license as NP/PA/MD/DO or eligible to obtain.
• Board certification in Family Medicine, Internal Medicine, or applicable specialty.
• Strong interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Comfortable working in a fast-paced, collaborative environment.
• Proficient in using electronic health records.
• FNP/AGNP credentials may be applicable.
• AGNP credentials (Adult Gerontological NP) as applicable.

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments.
• Perform comprehensive assessments, order labs/imaging, and diagnose acute and chronic conditions.
• Develop personalized treatment plans including lifestyle interventions, supplements, hormone therapies, and prescriptions.
• Educate and empower patients on preventive health and functional medicine principles.
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff.
• Document visits in the EHR in a timely and compliant manner.
• Participate in team meetings, quality improvement initiatives, and training sessions.
• Support clinic operations by contributing to a positive, team-oriented environment.
