Job summary

OnePeak Medical is seeking a passionate Medical Provider to join our Southern Oregon clinics in Talent, Medford, Central Point, Grants Pass, and White City. You will deliver comprehensive medical services, foster lasting patient relationships, and collaborate with a high-functioning care team to drive whole-person health outcomes.

We are hiring for several locations with a strong emphasis on preventive and functional medicine and patient education.

Qualifications

• Active Oregon license as NP/PA/MD/DO (or license eligibility).
• Board certification in Family Medicine or Internal Medicine.
• Experience in preventive, functional, and lifestyle medicine with strong patient communication.

Responsibilities

• Provide direct patient care through office visits, telehealth, and same-day appointments.
• Order and interpret labs/imaging; diagnose acute and chronic conditions.
• Develop personalized treatment plans including lifestyle interventions, supplements, hormones, and prescriptions.
• Educate and empower patients on preventive health and functional medicine concepts.
• Collaborate with health coaches, nutritionists, mental health providers, and clinical staff.
• Document visits in the EHR timely and accurately.
• Participate in team meetings, quality improvement, and training sessions.
• Support clinic operations with a positive, team-oriented approach.
