Job summary

Riverview Health in Minnesota is seeking a full-time SUD Outpatient Treatment Coordinator to provide coordination for individuals with substance use disorders. Responsibilities include service coordination, resource navigation, and care continuity.

Qualifications

• Must be 18 years of age or older.
• Ability to pass Minnesota Department of Human Services criminal background study.
• CPR/First Aid Certification preferred.

Responsibilities

• Provide non-clinical coordination for individuals with substance use disorders.
• Facilitate connections to medical, mental health, housing, employment, and recovery services.
• Assist with daily living skills and life skills development.
