Fairview Health Services in Minneapolis, MN is seeking a SUD Counselor to implement the treatment program for clients. You will work with the treatment team to plan, coordinate, and deliver SUD counseling services and build therapeutic relationships with clients and their families.

Responsibilities include conducting comprehensive assessments, developing treatment plans, facilitating counseling sessions, and ensuring documentation meets licensing and accreditation requirements.

Qualifications

• Has coursework or experience related to adolescent development (30 hours or 3 credit course).
• Minimum 150 hours of supervised experience as an adolescent counselor.
• No felony conviction in the last three years.
• No chemical use problem in the last two years.

Responsabilités

• Oversee and conduct comprehensive assessments for clients.
• Complete assessments and summarize client needs.
• Develop and implement individualized treatment plans.
• Involve client and family in treatment planning.
• Facilitate individual/group/educational sessions and family groups.
• Document care coordination activities following licensing requirements.
• Engage in evidence-based practice activities and continual learning.
