Job summary

Countryside Care Center in Macomb, IL is hiring a Speech-Language Pathologist (SLP) for a full-time role. The position focuses on evaluating and treating speech, language, cognitive, and swallowing disorders in a skilled nursing and post-acute setting.

You will collaborate with physicians, nursing staff, PTs, and families to provide coordinated care. You will develop individualized plans, deliver evidence-based therapy, monitor progress, and maintain regulatory-compliant documentation.

Qualifications

• Active Illinois Speech-Language Pathologist license required
• Excellent communication and clinical decision-making skills
• Experience in skilled nursing or rehabilitation is preferred, but not required
• New graduates are welcome to apply

Responsibilities

• Perform comprehensive speech-language, cognitive, and swallowing evaluations
• Design individualized treatment plans to promote functional independence
• Deliver evidence-based therapy for speech, communication, cognition, and dysphagia
• Monitor resident progress and update treatment plans as needed
• Collaborate with physicians, nursing staff, PTs, OTs, and families for coordinated care
• Maintain timely, accurate documentation compliant with regulations
