Job summary

Concept Rehab & Engage Consulting in Traverse City, Michigan is seeking a dedicated Speech-Language Pathologist (SLP) for a full-time role in a post-acute, skilled nursing setting to help adults and geriatric patients improve communication, cognition, and swallowing.

You will join an interdisciplinary rehabilitation team, receive clinical mentorship, and enjoy a stable schedule, competitive compensation, and comprehensive benefits.

Qualifications

  • Master’s degree in Speech-Language Pathology.
  • Current Michigan SLP license or licensure eligibility.
  • CCC-SLP certification preferred.
  • Strong communication, clinical reasoning, and organizational skills.
  • Experience in SNF, post-acute, inpatient rehab, long-term care, home health, or outpatient therapy preferred but not required.
  • New graduates and CFYs encouraged to apply.

Responsibilities

  • Evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders.
  • Develop individualized treatment plans based on patient needs and goals.
  • Provide evidence-based interventions to improve communication and swallowing safety.
  • Educate patients, families, and caregivers on treatment strategies and compensatory techniques.
  • Collaborate with PTs, PTAs, OTs, OTAs, nursing staff, physicians, and care teams.
  • Maintain timely and accurate documentation in compliance with Medicare and regulatory standards.
  • Participate in interdisciplinary care planning to support optimal patient outcomes.

What You'll Do

  • Strong communication, clinical reasoning, and organizational skills
  • Experience in Skilled Nursing (SNF), Post-Acute Care, Inpatient Rehabilitation, Long-Term Care, Home Health, or Outpatient Therapy is preferred but not required
  • New graduates and Clinical Fellows (CFY) are encouraged to apply
