Job summary

Concept Rehab, Inc. in Traverse City, MI is seeking a dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team in a full-time capacity.

You will evaluate and treat adults and geriatric patients with speech, language, cognitive-communication, and swallowing disorders in a post-acute/skilled nursing setting. You will develop individualized treatment plans, provide evidence-based interventions, and educate patients and families while collaborating with an interdisciplinary team.

Qualifications

• Master’s degree in Speech-Language Pathology.
• Current Michigan SLP license (or eligibility for licensure).
• Certificate of Clinical Competence (CCC-SLP) preferred.
• Strong communication, clinical reasoning, and organizational skills.
• Experience in SNF, post-acute care, inpatient rehab, long-term care, home health, or outpatient therapy is preferred but not required.
• New graduates and CFY are encouraged to apply.

Responsibilities

• Evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders.
• Develop individualized treatment plans based on patient needs and goals.
• Provide evidence-based interventions to improve communication and swallowing safety.
• Educate patients, families, and caregivers on treatment strategies and compensatory techniques.
• Collaborate with PTs, PTAs, OTs, OTAs, nursing staff, physicians, and care teams.
• Maintain timely and accurate documentation in compliance with Medicare and regulatory standards.
• Participate in interdisciplinary care planning to support optimal patient outcomes.
