Job summary

Concept Rehab, Inc. in Traverse City, Michigan, seeks a dedicated Speech-Language Pathologist (SLP) for a full-time role in our rehabilitation team. You will evaluate adults and provide treatment to improve communication, cognitive-communication, and swallowing.

Qualifications

• Master’s degree in Speech-Language Pathology.
• Current Michigan SLP license or eligibility.
• CCC-SLP certification preferred.
• Experience in SNF/post-acute care is preferred but not required.
• New graduates and CFY encouraged to apply.

Responsibilities

• Evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders
• Develop individualized treatment plans based on patient needs and goals
• Provide evidence-based interventions to improve communication and swallowing safety
• Educate patients, families, and caregivers on treatment strategies and compensatory techniques
• Collaborate with PTs, PTAs, OTs, OTAs, nursing staff, physicians, and care teams
• Maintain timely and accurate documentation in compliance with Medicare and regulatory standards
• Participate in interdisciplinary care planning to support optimal patient outcomes

What You'll Do

• Strong communication, clinical reasoning, and organizational skills
• Experience in Skilled Nursing (SNF), Post-Acute Care, Inpatient Rehabilitation, Long-Term Care, Home Health, or Outpatient Therapy is preferred but not required
• New graduates and Clinical Fellows (CFY) are encouraged to apply
