Ultimate Therapy in Baltimore, MD, is hiring a full-time Speech-Language Pathologist for Autumn Lake at Long Green. You will assess and treat adult patients with communication and swallowing disorders, coordinating with nursing and dietary staff to optimize outcomes.

Qualifications

• Active Maryland Speech-Language Pathologist license.
• Strong assessment and treatment skills in adult communication, cognition, and swallowing.
• Clear communication and accurate, timely clinical documentation.
• Geriatric rehabilitation or skilled nursing experience is a plus.

Responsibilities

• Evaluate speech, language, cognitive-communication, and swallowing function to determine services.
• Develop individualized plans of care with measurable goals reflecting residents' priorities.
• Provide treatment for communication and functional memory/attention/problem-solving challenges.
• Deliver dysphagia interventions and coordinate with nursing/dietary staff.
• Educate residents, families, and caregivers on strategies for participation outside therapy.
• Document clinical findings, reassess outcomes, and contribute to discharge planning.
