Palm Springs Post Acute and Healthcare Center is hiring a Social Worker for a full-time position in a compassionate, patient-centered setting. The role focuses on psychosocial assessment, care planning, and coordination across care teams to promote resident well-being.

Qualifications

• Bachelor’s degree required; Master’s degree strongly preferred.
• Minimum 1+ years of experience as a Social Worker in a healthcare-related setting.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.

Responsibilities

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs with ongoing communication.
• Build and maintain positive relationships with residents and families.
• Coordinate care with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans.
• Participate in care planning meetings to support continuity of care.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with facility policies, regulations, and documentation standards.
