This full-time role requires strong communication, teamwork, and knowledge of healthcare regulations; you will support residents and families, participate in care planning, and provide counseling and referrals.

Qualifications

• Bachelor’s degree required; master’s degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.

Responsibilities

• Evaluate residents to assess psychosocial needs and care planning.
• Address resident concerns and communicate with families and case managers.
• Build and maintain supportive relationships with residents and families.
• Coordinate care with interdisciplinary teams.
• Develop and update individualized psychosocial care plans.
• Participate in care planning meetings and ensure continuity of care.
• Provide counseling and resource referrals.
• Ensure compliance with facility policies and documentation standards.
