Palm Springs Post Acute and Healthcare Center is seeking a full-time Social Worker passionate about enhancing residents' quality of life. This position involves assessing residents' needs, developing care plans, and providing emotional support in a collaborative healthcare team.

The ideal candidate will have a Bachelor’s degree, strong interpersonal skills, and at least 1 year of healthcare-related experience.

Qualifications

• 1+ years of experience as a Social Worker in a healthcare-related setting.
• Strong knowledge of regulations governing nursing care facilities.
• Ability to work effectively in a fast-paced, team-oriented environment.

Responsibilities

• Evaluate residents' psychosocial needs and strengths.
• Address emotional needs while communicating with families.
• Build supportive relationships with residents and families.
• Coordinate care with interdisciplinary teams.
• Develop and update individualized psychosocial care plans.
• Participate in care planning meetings for continuity of care.
• Provide counseling and resource referrals as needed.
• Ensure compliance with facility policies and regulations.
