Job summary

Hampden at Post Acute and Healthcare Center in Wilbraham, MA is seeking a dedicated Social Worker for a full-time position. You will assess residents’ psychosocial needs, develop care plans, coordinate services, and support families to promote well-being and quality of life.

Qualifications

• Bachelor's degree required; master's degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; nursing home or rehabilitation center experience preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.

Responsibilities

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs while communicating with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident's stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.
