Job summary

Cape Cod Post Acute and Healthcare Center in Brewster, MA is seeking a full-time Social Worker to assess residents' psychosocial needs, develop care plans, and coordinate services to promote wellbeing.

Bachelor’s degree is required; Master’s degree is preferred. Minimum 1+ year experience in healthcare social work with strong communication and teamwork skills is expected, in a nursing home or rehab setting.

Qualifications

• Bachelor’s degree required; Master’s degree strongly preferred.
• Minimum 1+ years of experience as a Social Worker in a healthcare-related setting required; nursing home or rehabilitation center experience preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.

Responsibilities

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs with ongoing communication to families and case managers.
• Build and maintain positive relationships with residents and families.
• Coordinate care with interdisciplinary teams and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans.
• Participate in care planning meetings and support continuity of care.
• Provide counseling, emotional support, and referrals as needed.
• Ensure compliance with facility policies, regulations, and documentation standards.
