Job summary

Tenet Healthcare Corporation is seeking a Social Worker II to deliver psychosocial assessments and develop post-discharge plans for patients with complex social needs in a Palm Springs area hospital.

Under the supervision of the Social Work Manager/LCSW, you will coordinate services, refer to community resources, and maintain knowledge of regulatory requirements to support patients and families in hospital and clinic settings.

Qualifications

• CA LCSW license required.
• Excellent verbal, written and computer skills.
• Excellent organizational and communication skills.
• Knowledge of internal policies and State/Federal regulations related to discharge planning and social services.
• 5150 certification within 90 days of inception date into this position and maintained at all times.

Responsibilities

• Assess and plan for patients with complex social issues and spiritual/cultural considerations.
• Assist with adoptions, abuse, and neglect cases and report to authorities.
• Participate in Complex Case Review and daily patient care conferences.
• Coordinate post-discharge services and ensure follow-up.
• Keep Case Managers updated on variances to post-discharge plans.
