Job summary

ABL Health Care in San Jose is seeking a Full-Time Registered Nurse (RN) for Home Health to evaluate patients, plan care, and monitor progress across the region. You will work with the Clinical Director and field staff to deliver compassionate, quality home-based nursing.

Requirements include California RN license, home health or hospice experience, CA driver's license, and ability to travel. The role offers competitive benefits and a supportive team in a patient-focused environment.

Qualifications

Current Registered Nurse (RN) license to practice in California.
Experience in Home Health or Hospice care.
Current CA Driver's License.
Able to travel.

Responsibilities

Provide nursing care within the scope of practice, following the patient’s individualized care plan, in coordination with the Interdisciplinary Team.
Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to appropriate physicians or care team.
Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
Supervise Licensed Vocational Nurses (LVNs) when assigned.
Comply with all company policies and procedures to ensure high standards of care.
