Job summary

ABL Healthcare in Vacaville is seeking a full-time Registered Nurse (RN) for our Home Health team. You will evaluate patients, develop and implement care plans, and coordinate with the Clinical Director to uphold high standards of care in the field.

The role requires CA RN licensure, home health or hospice experience, a valid CA driver’s license, and ability to travel across regions to provide patient-centered care.

Qualifications

• Current RN license to practice in California (CA RN).
• Experience in Home Health or Hospice care.
• Valid CA Driver's License.
• Willingness to travel for patient care across regions.

Responsibilities

• Provide nursing care within the patient's care plan and Interdisciplinary Team coordination.
• Administer treatments, monitor patient conditions, and report changes to the physician or case manager.
• Maintain accurate clinical records using company devices.
• Supervise LVNs when assigned.
• Comply with company policies to ensure high standards of care.
