Heritage House Nursing & Rehabilitation Center is seeking a Full-Time Physical Therapy Assistant (PTA) to join our in-house team. You will provide person-centered care to help residents improve mobility and quality of life under PT supervision.

Responsibilities include administering prescribed PT treatments, documenting progress, and collaborating with the interdisciplinary team to optimize outcomes.

Qualificações

• Active and valid Texas PTA license.
• SNF or long-term care experience preferred.
• Knowledge of PT treatment practices and documentation.
• Experience with Net Health EHR preferred.
• Strong communication and teamwork skills.

Responsabilidades

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide skilled physical therapy services and interventions under the supervision of a Physical Therapist.
• Respond to requests for service by relaying information and referrals to the Physical Therapist.
• Implement and modify patients' plans of care under PT supervision.
• Measure and record patients' motor function, strength, and muscle performance.
• Monitor patient response to treatment and adjust approaches with the interdisciplinary team.
• Provide education to patients, family members, and caregivers.
• Ensure documentation is thorough and compliant with policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
