Job summary

Heritage House Nursing is seeking a full-time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will provide person-centered care to improve mobility, strength, and quality of life for residents under the supervision of a Physical Therapist.

The ideal candidate holds a Texas PTA license, has SNF or long-term care experience, and is proficient with Net Health documentation. Excellent teamwork and communication are essential.

Qualifications

• Licensed PTA in Texas or eligible to practice
• Experience in Skilled Nursing Facility (SNF) or long-term care preferred
• Knowledge of PT treatment practices and documentation requirements

Responsibilities

• Administer prescribed physical therapy treatments to relieve pain and improve function
• Provide services under the supervision of a Physical Therapist
• Document progress and communicate with PT and care team
• Modify care plans as directed by PT
• Monitor patient response and adjust interventions as needed
• Educate patients, families, and caregivers
• Attend required meetings and maintain compliance with policies
