Job summary

Outcome HC in New Jersey is seeking a full-time Physical Therapist to join our skilled nursing facility rehabilitation team. You will evaluate and treat residents in subacute and long-term care, focusing on mobility, balance, and functional skills to improve independence.

Qualifications

• Active Physical Therapist license in New Jersey (or eligible to obtain)
• New grads welcome with mentorship and training
• Experience in SNF, long-term care, or geriatrics is a plus
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of an interdisciplinary team

Responsibilities

• Evaluate and treat residents in subacute and long-term care with orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation
