Job summary

Berlin Health & Rehabilitation Center in Barre, Vermont, is hiring a Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. The role offers $42-$52 per hour and a patient-centered approach focused on improving function and independence.

New graduates are welcome to apply. Candidates must hold an active Vermont PT license and have strong interdisciplinary collaboration skills within a SNF or long-term care setting.

Qualifications

• Active and valid Physical Therapy license in Vermont.
• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.
• Current knowledge of physical therapy treatment practices, techniques, and interventions.
• Familiarity with Net Health or similar electronic documentation systems preferred.
• Strong clinical, communication, organizational, and interpersonal skills.
• Ability to work effectively with patients and residents with a variety of needs and abilities.
• Ability to collaborate effectively with an interdisciplinary healthcare team.
• Must be in good standing with all applicable licensing boards and regulatory agencies.

Responsibilities

• Evaluate patients and residents to determine PT needs and rehabilitation goals.
• Develop individualized treatment plans based on evaluation findings.
• Provide skilled PT interventions to improve mobility, strength, balance, and independence.
• Administer medically prescribed treatments per plan of care.
• Monitor progress and modify interventions as needed.
• Educate patients, families, and caregivers regarding treatment plans and safety.
