Qualifications

• Active and valid Vermont Physical Therapy license.
• Previous experience in a SNF or long-term care setting preferred.
• Knowledge of physical therapy treatment practices and interventions.
• Familiarity with Net Health or similar electronic documentation systems.
• Strong clinical, communication, organizational, and interpersonal skills.
• Ability to work with a diverse patient/resident population.
• Willingness to collaborate with an interdisciplinary healthcare team.
• Commitment to compassionate, patient-centered care.
• Must be in good standing with licensing boards.

Responsibilities

• Evaluate patients and residents to determine PT needs, functional limitations, and rehabilitation goals.
• Develop individualized treatment plans based on evaluations and patient goals.
• Provide skilled PT interventions to improve mobility, strength, balance, and independence.
• Administer prescribed treatments in accordance with the plan of care.
• Monitor progress and adjust interventions as needed.
• Educate patients, families, and caregivers on treatment plans, exercises, mobility and safety.
• Collaborate with nursing, therapy, physicians and other team members to coordinate care.
• Document evaluations, treatments, progress, and required clinical information.
• Maintain compliance with federal/state regulations, professional standards and facility policies.
• Participate in meetings and duties as assigned by the Director of Rehabilitation.

Physical Therapist (PT) – Full-Time

Berlin Health & Rehabilitation Center is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Barre, Vermont.

Pay: $42–$52 per hour

We provide person-centered care focused on helping residents and patients improve function, maintain independence, and achieve meaningful rehabilitation goals. Our interdisciplinary team works collaboratively to provide high-quality, compassionate care.

Physical Therapist Responsibilities

• Evaluate patients and residents to determine their physical therapy needs, functional limitations, and rehabilitation goals.

• Develop individualized treatment plans based on evaluation findings and patient goals.

• Provide skilled physical therapy interventions to improve mobility, strength, balance, coordination, function, and independence.

• Administer medically prescribed treatments in accordance with the established plan of care.

• Monitor patient progress and response to treatment and modify interventions as clinically appropriate.

• Provide education and guidance to patients, families, and caregivers regarding treatment plans, exercises, mobility, and safety.

• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team to coordinate patient care.

• Document evaluations, treatment sessions, progress, and other required clinical information accurately and timely.

• Maintain compliance with applicable federal and state regulations, professional standards, and facility policies.

• Participate in required meetings and other duties as assigned by the Director of Rehabilitation.

Physical Therapist Qualifications

• Active and valid Physical Therapy license in Vermont.

• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.

• Current knowledge of physical therapy treatment practices, techniques, and interventions.

• Familiarity with Net Health or similar electronic documentation systems preferred.

• Strong clinical, communication, organizational, and interpersonal skills.

• Ability to work effectively with patients and residents with a variety of needs and abilities.

• Ability to collaborate effectively with an interdisciplinary healthcare team.

• Commitment to providing compassionate, patient-centered care.

• Must be in good standing with all applicable licensing boards and regulatory agencies.
